If you are an ACTIVE member of the Defined Benefit Plan, please review the So You've Made the Decision to Retire handbook. Then you must submit an Estimate of Retirement Benefits Request form to our office so that you can see what you are looking to receive in retirement. Once you have received your numbers and would like to speak with a retirement counselor, please call 804-646-5958 to set up an appointment. If you do not need to speak with a counselor, you can submit the following forms to our office 60 to 90 days prior to your retirement date.

- Beneficiary Appointment/Change
- Certification Form
- Health/Dental Authorization form
- Direct Deposit Authorization form
- Federal Tax Withholding Certificate
- Virginia State Tax Withholding Certificate
If you are an SWORN member/employee of the Defined Benefit Plan AND you are eligible to enter the DROP (Deferred Retirement Option Program) , please review the DROP Administrative Guidelines. Then you must submit to our office an Estimate of Retirement Benefits Request form so that you can see what you are looking to receive in retirement. Once you have received your numbers and would like to speak with a retirement counselor, please call 804-646-5958 to set up an appointment. If you do not need to speak with a counselor, you can submit the following forms to our office 60 days prior to your DROP Entry retirement date.
